Preventive and comprehensive dental services through Delta Dental®

Find a dentist near you

Go to deltadentalmi.com/find-a-dentist. Choose Medicare Advantage PPO, put in your location and click Search. You will then see a list of available dentist offices near you to choose from and schedule an appointment. You can also call Delta Dental at 800.330.2732 (TTY 711). 

  • $0 copay for two preventive dental exams per year
  • $0 copay for two cleanings (regular or periodontal maintenance) per year
  • $0 copay for one fluoride treatment per year
  • $0 copay for one set (up to 4 films in a single visit) of bitewing x-rays per year
  • $0 copay for one brush biopsy per year
  • $0 for periapical radiographs, as needed
  • $0 for radiographs (full-mouth or panoramic x-rays) once every 24 months
  • $0 for two periodontal maintenance cleanings (two total each year)
  • $1,500 annual maximum on all covered dental services

Go to eyemed.com/selectprovider. Enter your Zip Code and click Search. You will then see a list of available eye care locations near you to choose from and schedule an appointment. You can also call EyeMed at 844.366.5127 (TTY 711).

  • $0 for one routine exam each year (including dilation and refraction)
  • $0 for one retinal imaging per year
  • $200 for eyewear, such as glasses or contacts

Call TruHearing to speak to a hearing consultant at 833.714.5355 (TTY 711), Monday through Friday from 8 a.m. to 8 p.m. 

  • $0 routine hearing exam per year
  • One advanced-level hearing aid per ear, every two years
  • $0 for all follow-up provider visits within the first year of hearing aid purchase
  • 80 batteries per hearing aid for non-rechargeable models
