Vision, dental and hearing coverage

Your plan includes routine vision from EyeMedSM providers, hearing coverage from TruHearingTM providers and preventive and comprehensive dental services through Delta Dental®.

Drug coverage

This plan has no deductible, so you'll pay these amounts for your drugs.
All Part D Prescription drugs. $0

You will receive your annual Low-Income Subsidy (LIS) letter that explains your LIS level and copay amounts for your plan year. This is also known as Extra Help. Since you qualify for Extra Help your copay for all Part D prescription drugs is $0.

Medication Therapy Management

Keep up to date on the medications you need with our Medication Therapy Management (MTM) program. We work with specially trained area pharmacists to help you get the best results from your medications while keeping your out-of-pocket costs down. These pharmacists do much more than dispense drugs: they review your current medications and go over any questions you may have.
