Priority Health Connect available to all members
On April 21, we expanded Priority Health Connect to all members.
This free tool allows your patients to instantly search for the resources that they need within their zip code, including services like housing support, legal assistance, COVID-19 programs, access to healthy foods, and childcare. Agencies across the state can list their free or low-cost services for free within the platform. There are more than 6,800 programs currently available in the state of Michigan on Priority Health Connect.
Priority Health also provides an incentive to practices that screen patients and submit information on Social Determinants of Health (SDoH). According to the Center for Disease Control, social SDoH are the conditions in the places where people live, learn, work, and play that affect a wide range of health and quality of life outcomes.
This tool can be used as a resource providers offer their patients identified through this measure.
