All contracted providers should use our online portal to submit post-claims appeals and medical records. Since 2020, we've been making changes to improve your experience and give you more timely resolution. That’s why we're moving to an online process. You'll no longer be able to submit appeals via fax or mail.

Refer to our appeals page for simple submission instructions. Just select if the claim is commercial, Medicare, or Medicaid and follow the steps for submitting your appeal.
