Legacy THC providers: How to submit your Healthy Michigan members' HRA forms
Following our merger with Total Health Care (THC), below are instructions for legacy THC providers on how to submit their Healthy Michigan patients’ Health Risk Assessments (HRAs) and how their incentive payments will be made:

This doesn’t change our standard PCP Incentive Program (PIP) process for HRAs. Providers participating in PIP must submit by the PIP process and deadline on Feb. 28, 2022.
Legacy THC providers won’t participate in PIP until Jan. 1, 2022. At that time, all providers will follow the PIP process.
Why is this important?
The HRA completion rate is a bonus measure that, for each qualifying HRA, earns providers $25 and members reduced cost share on their health plan. HRAs not submitted following the instructions and deadlines above will not count in 2021.
