Priority Health expands impactful diabetes management program with virtual support program
(GRAND RAPIDS, Mich. — May 31, 2023) Priority Health, a nationally recognized nonprofit health plan, has expanded its high-performing diabetes management program to support members with type 2 diabetes.
The American Diabetes Association reports 35 million Americans – 1 in 10 – are living with type 2 diabetes, and many will experience at least two other chronic conditions as a result. Priority Health can better help people manage their disease by providing access to the Livongo by Teladoc Health diabetes management program that includes expert coaching, monitoring tools and lifestyle interventions tailored to best suit each member’s needs and optimize their health outcomes. The program is now available for Priority Health members who are enrolled in a group health plan.
Livongo is the latest tool to join Priority Health’s diabetes management program for employer groups – an initiative that has resulted in top health outcomes compared to regional and national benchmarks. Priority Health continuously ranks No. 1 among Michigan health plans for well-controlled HbA1C (the simple test that measures blood sugar levels) and among the top 5% nationally.

Members who use the Livongo by Teladoc Health diabetes management program have access to a web portal and mobile app where they can access live coaching, diabetes education, emotional health monitoring tools and information on nutrition and exercise. Members who enroll in the program will receive a kit that includes a blood glucose meter that offers real-time feedback and is connected to the platform.
